Beginning with the 2012-2013 school year, 7th and 12th grade students must have Tetanus, Diphtheria, Acellular Pertussis and Meningococcal immunizations before they are allowed to come to school.

In order to attend one of the School Based Health Center Immunization Clinics, students should be accompanied by an adult, bring their health insurance, and complete an enrollment form that is available on site or for download below:
